Nadine" sank off the Madeleine Islands, Quebec, in high winds and three metre high waves, causing death by hypothermia of seven crew members and a fisheries researcher: there were two survivors: a coroner's report released in June 1992 blamed the loss of life and sinking of the vessel on a series of human errors: ill-fitting survival suits, malfunctioning navigational equipment, lack of automatic pumps, lack of safety measures and emergency drills for those aboard the vessel, and ship's officers untrained in emergency procedures. |
